Régime de l’assurance maladie du Québec (RAMQ)
If you’ve just arrived from another province, you must register for the régime d’assurance maladie as soon as possible after your move. To register call the régie at 1 800 561-9749. Forms will be sent to you by mail.
They will ask you to check the information and return it with the required documentation:
- An original birth certificate
- Two photos
- Letter that certifies the date of your arrival for posting
- Sworn statement
Please note that if you are married and have chosen to take on your spouse’s surname, your health insurance card and driver’s licence will be issued with your maiden name.
** Please note that since June 18, 2012, family members (spouse or spouse and children) of Canadian Forces servicemen or servicewomen who come from territories or provinces other than Quebec and who have the status that allows them to settle here become admissible for health insurance as soon as they arrive in Quebec, since they are exempted from the three-month waiting period. To prove their situation the family must provide the document issued by the Canadian Forces in which their instructions and posting message are included.
Children are covered by the régie until their 18th birthday when their parents are eligible for coverage. They must then register as adults. If the child is in school full-time, living with his or her parents and does not have a spouse, he or she is covered until the age of 25 years or until he or she stops studying full-time. It isn’t necessary to include photos with your registration form for children under 14 years of age.
